Some services provided are not covered under our contract with the NHS and therefore will be charged. Fees are based on the British Medical Assosiation (BMA). You can speak to the practice secretary by calling the practice on 0121 465 2888 option 3 between Monday, Tuesday, Thursday and Friday 09:30-15:00.

We can provide the following non NHS Services:

• Passport signing please note you must have been registered with the practice for 2 years or more. For a child under the age of 2 the parent/carer must have been registered with us for that time.
• Citizienship forms please note you must have been registered with the practice for 2 years or more. For a child under the age of 2 the parent/carer must have been registered with us for that time.
• Insurance claim forms.
• HGV, PSV, taxi medicals.
• Private letters.
